Roles & Responsibilities
- Assess patients’ functional mobility and develop individualized treatment plans to restore independence
- Deliver hands-on and evidence-based therapeutic interventions to improve strength, balance and mobility
- Collaborate with physicians, nurses and the interdisciplinary team to coordinate safe, timely care
- Document progress accurately in the charting system and maintain clear, timely records
- Educate patients and families on home exercise programs and strategies for safe discharge
- Participate in discharge planning to support effective transitions of care and reduce readmissions
- Maintain compliance with hospital policies, safety protocols and infection control standards
Qualifications
- Physical Therapy license (eligible to submit required)
- Minimum 2 years of experience in acute care or hospital-based physical therapy
- BLS (Basic Life Support) certification required
- Comfortable using PointClickCare or similar electronic charting systems
- Strong clinical assessment, manual handling and patient-education skills
- Available to begin the assignment starting 2025-12-29
Knowledge & Skills
- Acute care physical therapy best practices and safety-focused interventions
- Patient assessment, treatment planning and outcome measurement
- Proficient documentation in electronic health record systems (PointClickCare experience a plus)
- Excellent communication and collaboration with interdisciplinary teams
- Skilled in mobility training, transfers, gait training and ADL retraining
- Ability to prioritize caseloads and respond to changing patient acuity
