Roles & Responsibilities

• Provide skilled physical therapy services to residents in a skilled nursing setting with compassion and clinical excellence
• Assess patient function, create individualized treatment plans, and set meaningful, measurable goals
• Deliver therapeutic exercise, balance and gait training, manual therapy, and mobility interventions
• Monitor and document patient progress, adjusting treatment based on outcomes and team input
• Collaborate with nursing, occupational therapy, speech therapy, social work, and families to support safe discharges and quality of life
• Educate patients and families on home programs, fall prevention, and long-term mobility strategies
• Maintain timely, accurate documentation in the facility EMR and comply with regulatory standards

Qualifications

• Active California Physical Therapy license (required)
• Current BLS/CPR certification (required or obtain upon hire)
• Experience in skilled nursing, rehab, or geriatric settings preferred
• Strong clinical reasoning, documentation skills, and ability to work within an interdisciplinary team
• Willingness to adapt to facility policies, pass background checks, and complete required onboarding

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy accepted

Knowledge & Skills

• Knowledge of geriatric rehabilitation principles and chronic condition management
• Skilled in gait training, transfers, balance training, and therapeutic exercise prescription
• Familiarity with fall risk assessment and prevention strategies
• Comfortable using electronic medical records for documentation and outcome tracking
• Excellent communication skills for patient/family education and team collaboration
• Ability to prioritize caseloads and manage time effectively in a busy SNF environment
