Roles & Responsibilities
- Provide compassionate direct resident care including medication administration and assessments
- Collaborate with interdisciplinary team members to develop and implement individualized care plans
- Document accurately and timely in the medical record to ensure continuity of care
- Monitor residents’ conditions and promptly report changes to the RN or provider
- Support a safe, clean and therapeutic environment that promotes resident dignity and comfort
- Assist with admissions, transfers and discharges while maintaining excellent communication with families
Qualifications
- Current Licensed Practical Nurse (LPN) license in New Mexico or compact/eligible to obtain
- Basic Life Support (BLS) certification preferred or willing to obtain upon hire
- Current immunizations and TB screening per facility policy
- Prior LPN experience in long-term care or rehabilitation settings preferred
- Strong communication skills and a patient-centered approach
Education
- Graduate of an accredited LPN/Vocational Nursing program
- High school diploma or GED
Knowledge & Skills
- Skilled in medication administration and basic clinical assessments
- Comfortable with charting and following care plans
- Knowledge of infection control practices and resident safety protocols
- Ability to work independently on the night shift and escalate concerns appropriately
- Team player who brings empathy, reliability, and good judgment
