Candidate Pay Rate: $68 per hour

Roles & Responsibilities
- Perform comprehensive in-home physical therapy evaluations and create individualized treatment plans
- Deliver compassionate, goal-driven care across age groups from pediatrics through geriatrics
- Document thoroughly and accurately, including required OASIS assessments and timely clinical notes
- Coordinate care with nurses, OTs, social workers and hospice team members to optimize outcomes
- Educate patients and families on safe mobility, fall prevention, and home exercise programs
- Provide skilled support in palliative and hospice contexts when appropriate, honoring patient goals
- Manage a rural caseload safely — plan routes, drive in winter conditions, and prioritize patient safety
- Maintain current PT skills, participate in required onboarding, and complete requested checklists and references
Qualifications
- Active New Hampshire Physical Therapy license (Compact allowed)
- Current American Heart Association BLS (required, submit documentation)
- 2+ years current experience in Physical Therapy in Home Health (required)
- Home Health and/or Hospice experience with documented OASIS competency (required)
- Pediatric to geriatric experience across the lifespan
- Previous travel experience highly preferred
- Own reliable vehicle and comfortable driving in winter conditions
- Must live 50+ miles from the facility (no local candidates) and include permanent address in submission
- Submission packet must include: up-to-date work history, PT skills checklist (within past year), and one supervisory signed reference from past year
Education
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
Knowledge & Skills
- Strong OASIS assessment and documentation skills
- Skilled in home-based functional assessments and mobility interventions
- Comfortable treating diverse diagnoses across pediatric to geriatric populations
- Excellent communication and patient/family teaching abilities
- Ability to collaborate within an interdisciplinary home health and hospice team
- Efficient caseload and time management for rural route scheduling
- Safe patient handling and fall prevention strategies
- Adaptability to changing in-home environments and patient needs
