Roles & Responsibilities

• Provide in-home physical therapy services to maximize patient independence and safety
• Assess patient mobility, function, and home safety and create individualized treatment plans
• Deliver skilled therapeutic interventions focused on functional goals and quality of life
• Coordinate care with nurses, aides, social workers, and hospice team members
• Educate patients and caregivers on safe techniques, home exercise programs, and fall prevention
• Document timely, clear, and thorough clinical notes and progress toward goals
• Work independently in the field while engaging collaboratively with the multidisciplinary team

Qualifications

• Active Vermont Physical Therapist license required
• Valid driver’s license and reliable vehicle required for in-home visits
• Experience in home care or hospice settings is preferred
• Comfortable working independently and managing a travel caseload
• Strong communication and teamwork skills for multidisciplinary collaboration

Knowledge & Skills

• In-home therapy best practices and home safety assessment
• Patient assessment and creation of individualized care plans
• Care coordination with home health and hospice teams
• Experience with documentation and clinical reporting for home health
• Ability to prioritize visits and manage time while traveling between patients
