Roles & Responsibilities

• Assess patients regularly using the nursing process and provide skilled nursing care in line with physician orders and agency policies
• Initiate, revise and adapt individualized plans of care to meet changing patient needs
• Provide medication management and evaluate therapeutic responses to ensure safe, effective treatments
• Coordinate patient care and evaluate outcomes, communicating necessary changes to the care team
• Work closely with CNAs and the interdisciplinary team to ensure seamless, patient-centered care
• Notify physicians of changes in patient condition and obtain new orders when necessary
• Document care thoroughly and accurately in Epic to support continuity and quality of care

Qualifications

• Active Michigan RN license (LARA and/or Nursys verification required)
• Minimum 2+ years of Long Term Care nursing experience (required)
• Current American Heart Association BLS certification (required)
• Current skills checklist pertinent to LTC (within 1 year) — must be provided with submission
• Two supervisory reference forms completed within the last two years (required)
• Willingness to work rotating night shifts and follow facility scheduling rules
• Candidates must provide permanent address at submission — facility will not consider local candidates within a 100-mile radius

Knowledge & Skills

• Patient assessment and clinical judgment across common LTC conditions
• Care planning, coordination and documentation using Epic
• Medication management and monitoring therapeutic responses
• Strong communication and teamwork with CNAs and interdisciplinary staff
• Familiarity with LTC nursing process, standards of care and regulatory requirements
• Ability to prioritize, adapt care plans and escalate changes to providers
