Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in the rehab setting
• Administer medications and treatments safely and document accurately
• Perform focused patient assessments and report clinical changes promptly
• Assist with activities of daily living and support patient independence
• Collaborate with RN colleagues, therapists, and care team members to create and follow care plans
• Maintain infection control standards and ensure a safe environment for patients and staff
• Educate patients and families on care plans, medication instructions, and discharge needs

Qualifications

• Active LPN license in New Mexico (required)
• Completion of an accredited practical/vocational nursing program
• Current BLS/CPR certification or willingness to obtain promptly
• Comfort working in a rehab/long-term care environment; rehab experience is a plus
• Valid background check and required immunizations as per facility policy
• Strong interpersonal skills and a team-first attitude

Education

• Graduate of an accredited practical/vocational nursing program (LPN/LVN)

Knowledge & Skills

• Solid knowledge of medication administration and basic clinical procedures
• Familiarity with rehab patient needs and supportive care approaches
• Accurate clinical documentation and electronic charting skills
• Good patient assessment and vital-sign monitoring abilities
• Clear communication with patients, families, and interdisciplinary teams
• Time management and prioritization during busy shift periods
