Roles & Responsibilities

• Provide compassionate, hands-on care to patients by assisting with activities of daily living
• Monitor and record vital signs and patient conditions accurately and promptly
• Support mobility needs safely — transfers, turning, and fall prevention
• Follow individualized care plans and communicate changes to the nursing team
• Maintain clean, safe patient environments and adhere to infection control practices
• Document care activities thoroughly and uphold patient confidentiality
• Collaborate with nurses and therapists to deliver timely, coordinated care

Qualifications

• Current Licensed Nursing Assistant (LNA) certification — VT or NH licensure indicated
• Recent experience in long-term care or similar clinical setting preferred
• Current basic life support (BLS/CPR) certification preferred
• Strong communication and teamwork skills, with a patient-first attitude
• Ability to work night shifts (7:00 PM - 5:00 AM) and follow scheduled assignment dates

Education

• High school diploma or GED
• Completion of an approved Nursing Assistant training program and successful LNA certification

Knowledge & Skills

• Solid understanding of activities of daily living (ADLs) and patient hygiene care
• Proficient in taking and recording vital signs and reporting changes
• Familiarity with electronic and paper charting procedures
• Knowledge of infection prevention and safety best practices
• Good interpersonal skills for working with patients, families and interdisciplinary teams
