Roles & Responsibilities
- Assess patients and families using a holistic approach to identify social, emotional, and environmental needs
- Provide crisis intervention and supportive counseling to patients and their families
- Coordinate discharge planning and community resources to promote safe transitions of care
- Document assessments, care plans, and interventions clearly and promptly in the medical record
- Advocate for patients' needs with clinical teams, community agencies, and families
- Deliver culturally sensitive care, engaging patients and families in meaningful ways
- Supervise or cover supervisory duties as needed to ensure continuity of social services
Qualifications
- MSW (Master of Social Work) from an accredited school of Social Work
- Licensed in Alaska at the Master level (LMSW - Alaska required)
- Minimum one (1) year experience in a hospital or health-care setting
- Valid Alaska state driver’s license
- Basic computer skills for documentation and communication
- Preferred: cross-cultural experience and bilingual English & Yup'ik language skills
- Ability to present information clearly in one-on-one and group settings
- Mathematical competency for basic computations and data interpretation
Education
- MSW (Master of Social Work) from an accredited school of Social Work
Knowledge & Skills
- Medical social services and hospital social work practice
- Case management and care coordination
- Assessment and treatment planning
- Clear, compassionate communication with patients, families, and care teams
- Accurate documentation and clinical record keeping
- Ability to work independently and perform supervisory tasks when needed
- Cultural humility and experience in cross-cultural care
