Shift Type: Sunday-Thursday, 8:00 AM - 4:30 PM (Day); every other weekend required; floating expectation to Laguna facility; schedule may be adjusted based on staffing

Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Conduct thorough biopsychosocial assessments and document findings in EPIC to guide care
- Develop individualized discharge plans and coordinate post-acute services to promote safe transitions
- Complete initial assessment within 24 hours of admission to move the plan forward quickly
- Collaborate with the medical team, insurance case managers, and external partners to secure resources and placements
- Provide crisis intervention and contingency planning when patients face urgent psychosocial needs
- Refer patients to rehab, long-term care, community resources, and social service programs
- Maintain clear, timely documentation of care delivery, goals, teaching, and patient responses
- Participate in care conferences and facilitate effective interdisciplinary communication
Qualifications
- Active professional social work license required upon start; pending licensure may be considered during application but must be active by start date
- Minimum 2 years of professional experience, with at least 1 full year of acute care experience required
- Travel experience required; first-time travelers will not be accepted
- Experience with Epic charting and EPIC hardstop workflows required
- BLS certification required; LCSW and MSW credentials preferred
- Comfort managing caseloads of ~20-23 patients and working in fast-paced acute care settings
- Ability to work every other weekend and follow RTO restrictions (no more than 7 days)
Education
- Master of Social Work (MSW) preferred
- LCSW credential preferred
- BLS certification
Knowledge & Skills
- Discharge planning and case management techniques for short term acute care
- Biopsychosocial assessment and development of problem lists and goals
- Knowledge of community programs and resources (Medicaid, Medicare, AFDC, TANF, HCBS, Veterans programs)
- Familiarity with protective services processes (adult/child protective services, guardianship, emergency detention)
- Care coordination across internal teams and external post-acute providers
- Crisis intervention, counseling, and contingency planning skills
- Understanding of major disease processes and their psychosocial impacts (CHF, COPD, dementia, diabetes, cancer, stroke, psychiatric disorders)
- Experience in or comfort with Trauma Level I/II environments and various units (ICU, L&D, NICU, Pediatrics, ED, Medical-Surgical) as preferred
