Roles & Responsibilities

• Provide safe, patient-centered inpatient pharmacy care across acute care populations including neonates through geriatrics
• Prepare and verify sterile and intravenous medications using sterile technique and compounding best practices
• Use Epic to document medication interventions, charting, and clinical communication accurately and efficiently
• Operate and manage medication dispensing systems such as Pyxis to ensure timely delivery of medications
• Perform therapeutic medication monitoring and clinical dosing adjustments (anticoagulation, renal, pediatric, pharmacokinetic dosing)
• Serve as the on-call pharmacist during your on-rotation week and respond to after-hours needs as required
• Collaborate with physicians, nurses, and the interdisciplinary team to optimize medication therapy and patient outcomes
• Participate in orientation and local process improvement activities to support pharmacy operations

Qualifications

• Minimum 1 year of pharmacy experience (inpatient/acute care experience preferred and explicitly requested)
• Experience with Epic charting (required)
• Familiarity with medication dispensing systems (Pyxis preferred)
• IV Certification preferred
• RPh (pharmacist licensure) preferred or active pharmacist license in state of practice
• Comfort with block scheduling and providing on-call coverage during 7-day on rotations
• Strong clinical dosing experience (anticoagulation, pediatric, renal, pharmacokinetic dosing preferred)
• Ability to complete a 40-hour orientation

• Medication dispensing systems and Pyxis operation
• Compounding and sterile technique for IV preparation
• Clinical dosing and therapeutic drug monitoring (anticoagulation, warfarin, renal dosing, pediatric dosing)
• Strong communication and interdisciplinary collaboration skills
• Time management for block schedules and on-call responsibilities
