Roles & Responsibilities

• Provide thorough outpatient physical therapy evaluations and develop individualized treatment plans
• Deliver hands-on and evidence-based interventions to improve patient mobility, function, and quality of life
• Coordinate care and communicate effectively with physicians, nursing staff, and other rehabilitation team members
• Document clinical findings, progress notes, and discharge plans accurately and timely
• Participate in Trinity Specialty Specific Competency Testing and complete the Trinity Skills Checklist as required
• Cover occasional Saturday clinic days as scheduled to ensure patient continuity of care

Qualifications

• Current Oregon PT license (required)
• BLS certification (required)
• Experience in outpatient physical therapy; some inpatient experience is a plus
• Availability for day shift hours (7:30 AM - 6:00 PM) and occasional Saturdays
• Successful completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist as part of onboarding
• Ability to pass required health screenings: Trinity 10-panel drug screen, fit test, Tdap, COVID-19 vaccine, MMR, Hepatitis B, influenza vaccine, 2-step PPD or IGRA, varicella, and physical/health statement

Knowledge & Skills

• Strong skills in outpatient evaluation, therapeutic exercise, and manual techniques
• Familiarity with inpatient transitions and cross-setting care coordination
• Excellent clinical documentation and time-management skills
• Patient education, fall prevention, and mobility training expertise
• Comfortable with competency-based testing and skills verification processes
• Effective communication and teamwork within multidisciplinary care teams
