Shift Type: Monday-Friday, 7:30 AM - 6:00 PM (Day), occasional Saturday coverage (typically 2 Saturdays per contract)
Hours per Week: 40
Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Provide thorough outpatient physical therapy evaluations and develop individualized treatment plans
- Deliver hands-on and evidence-based interventions to improve patient mobility, function, and quality of life
- Coordinate care and communicate effectively with physicians, nursing staff, and other rehabilitation team members
- Document clinical findings, progress notes, and discharge plans accurately and timely
- Participate in Trinity Specialty Specific Competency Testing and complete the Trinity Skills Checklist as required
- Cover occasional Saturday clinic days as scheduled to ensure patient continuity of care
Qualifications
- Current Oregon PT license (required)
- BLS certification (required)
- Experience in outpatient physical therapy; some inpatient experience is a plus
- Availability for day shift hours (7:30 AM - 6:00 PM) and occasional Saturdays
- Successful completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist as part of onboarding
- Ability to pass required health screenings: Trinity 10-panel drug screen, fit test, Tdap, COVID-19 vaccine, MMR, Hepatitis B, influenza vaccine, 2-step PPD or IGRA, varicella, and physical/health statement
Knowledge & Skills
- Strong skills in outpatient evaluation, therapeutic exercise, and manual techniques
- Familiarity with inpatient transitions and cross-setting care coordination
- Excellent clinical documentation and time-management skills
- Patient education, fall prevention, and mobility training expertise
- Comfortable with competency-based testing and skills verification processes
- Effective communication and teamwork within multidisciplinary care teams
