Roles & Responsibilities
- Provide compassionate direct patient care and essential nursing support
- Administer medications and treatments according to facility policies
- Perform timely and accurate patient assessments and document findings
- Collaborate with the interdisciplinary team to coordinate care plans
- Monitor patient condition and promptly report changes to the RN or provider
- Support resident comfort and safety while promoting dignity and independence
- Maintain accurate charting in the electronic medical record system
- Participate in facility initiatives to improve patient outcomes and quality
Qualifications
- Active Licensed Practical Nurse (LPN) license in New Mexico required
- Current BLS/CPR certification required or willingness to obtain
- Ability to pass background check and meet facility immunization requirements
- Comfortable working overnight shifts and in long-term care/rehabilitation settings
- Strong communication skills and a team-first attitude
- Previous LPN experience in a clinical setting preferred
Education
- Completion of an accredited Practical Nursing program (diploma or certificate)
Knowledge & Skills
- Medication administration and safe injection practices
- Basic patient assessment and vital signs monitoring
- Wound care fundamentals and infection control practices
- Charting and documentation accuracy in electronic medical records
- Time management and prioritization during busy shifts
- Compassionate patient interactions and family communication
