Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents at Laconia Rehab Ctr
• Administer medications and treatments accurately and document thoroughly
• Perform head-to-toe assessments and report changes in condition promptly
• Collaborate with interdisciplinary team members to build and follow care plans
• Support resident mobility, ADLs, and restorative care with dignity and respect
• Communicate clearly with families and the care team to ensure continuity

Qualifications

• Licensed Practical Nurse (LPN) — current and active licensure required
• Comfortable working overnight shifts (7:00 PM - 7:30 AM)
• Experience in long-term care, rehabilitation, or skilled nursing preferred
• Strong clinical judgment, documentation skills, and a resident-centered approach
• Ability to pass facility-required background and health screenings

Education

• Completion of an accredited LPN/LVN training program; high school diploma or equivalent

Knowledge & Skills

• Medication administration and accurate charting
• Basic wound care and infection prevention practices
• Patient assessment and vital sign monitoring
• Excellent communication and teamwork in a rehab/skilled nursing setting
• Time management and prioritization during busy night shifts
