Shift Type: 3:00 PM - 11:30 PM (Evening shift; specific days to be confirmed by facility)
Candidate Pay Rate: $42 per hour

Roles & Responsibilities
- Provide compassionate direct patient care including vital signs, medication administration, and wound care
- Perform timely and accurate documentation to support continuity of care and effective team communication
- Collaborate with nurses, therapists and providers to develop and update individualized care plans
- Monitor patient status and promptly report changes to the care team to ensure safety
- Assist with ADLs (activities of daily living) while encouraging patient independence and dignity
- Support a positive care environment by mentoring teammates and contributing to unit goals
Qualifications
- Current Licensed Practical Nurse (LPN) credential in good standing
- Travel LPN experience preferred; adaptable and ready to integrate into new teams
- Current BLS/CPR certification (or willing to obtain prior to start)
- Ability to pass facility background checks and required health screenings
- Strong communication skills and a patient-first attitude
Education
- LPN diploma or certificate from an accredited practical/vocational nursing program
Knowledge & Skills
- Medication administration and accurate charting
- Basic assessment skills and ability to recognize changes in patient condition
- Familiarity with infection control protocols and safety standards
- Effective time management and prioritization on a busy unit
- Comfortable using electronic medical records and facility documentation systems
