Roles & Responsibilities

• Assess patients' functional mobility and rehabilitation needs with thorough evaluations
• Develop and implement individualized treatment plans to restore independence
• Provide skilled therapeutic interventions using evidence-based practice
• Document patient progress clearly and timely in the medical record
• Collaborate with nurses, physicians, and the interdisciplinary team to coordinate care
• Educate patients and families on home programs and safety to support long-term outcomes
• Maintain professional licensure, certifications, and comply with facility requirements

Qualifications

• Active Physical Therapist license in California or eligibility to obtain
• BLS certification required
• DCI/BVP certification required
• Successful license/certificate verification completed
• Completion of Prime Core Competencies Exam, Prime Skills Checklist, and Prime Specialty Exam as required
• Up-to-date immunizations and health screenings (COVID-19, MMR, TB test, Hep B, Influenza, Varicella, Tdap, drug screen, fit test)
• Comfort working in a fast-paced rehab environment and collaborating with a multidisciplinary team

Knowledge & Skills

• Clinical skills in physical therapy and rehabilitation
• Proficiency with rehab assessment and treatment planning
• Experience with documentation and regulatory compliance
• Familiarity with required certifications and competency testing
• Strong patient communication and education skills
• Ability to perform safe, effective hands-on therapy and therapeutic exercise instruction
