Shift Type: Per diem days; schedule varies based on facility needs (day shifts only, specific start/end times assigned per shift)
Candidate Pay Rate: $42 per hour

Roles & Responsibilities
- Assist patients with activities of daily living, including bathing, dressing, grooming, and feeding
- Monitor and record vital signs and report changes to the nursing team promptly
- Provide safe and compassionate patient transfers and mobility support
- Maintain clear, timely documentation using facility forms and checklists
- Support infection control practices and adhere to PPE and hygiene protocols
- Participate in Prime Core Competencies and skills checklists to demonstrate clinical proficiency
- Communicate effectively with patients, families, and interdisciplinary staff to promote coordinated care
- Help maintain a clean, organized, and safe patient environment
Qualifications
- Current Certified Nursing Assistant (CNA) certification
- Basic Life Support (BLS) certification required
- DCI/BVP certification as listed by the facility
- Completion or ability to complete Prime Core Competencies Exam, Prime Skills Checklist, and Prime Specialty Exam
- Successful license/certificate verification
- Ability to meet health and screening requirements including COVID-19 vaccine, MMR, TB testing, drug screen, fit test, Hepatitis B, Influenza, Varicella, and Tdap
Education
- Certified Nursing Assistant (CNA) certification (state-approved)
Knowledge & Skills
- Strong patient care and bedside support skills
- Basic understanding of vital signs, patient mobility, and infection control
- Clear verbal and written communication for accurate documentation
- Comfort working in fast-paced acute care or long-term care settings
- Dependability, compassion, and teamwork mindset
- Familiarity with Prime skills and competency-based assessments
