Roles & Responsibilities

• Assess patients’ functional abilities and design individualized treatment plans to promote independence
• Deliver skilled therapeutic interventions that address ADLs, adaptive equipment needs, and environmental modifications
• Collaborate with nurses, PTs, SLPs, physicians, and family members to coordinate holistic care
• Document patient progress clearly and timely, using facility-required systems and best practices
• Provide patient and caregiver education to reinforce home programs and promote safe transitions
• Participate in goal-setting meetings and discharge planning to ensure continuity of care
• Champion best practices and contribute to quality improvement initiatives within the rehabilitation team

Qualifications

• Valid Occupational Therapist license in California (or eligibility to obtain prior to start)
• Current BLS/CPR certification (or ability to obtain before start)
• Experience working in rehabilitation, outpatient, or post-acute settings preferred
• Strong clinical reasoning and documentation skills with patient-centered approach
• Excellent communication and teamwork skills; able to collaborate across disciplines
• Willingness to work an assignment covering all scheduled days as required

Education

• Master’s or Doctoral degree in Occupational Therapy (MOT/OTD) from an accredited program

Knowledge & Skills

• Skilled in ADL retraining, adaptive equipment recommendations, and energy conservation techniques
• Knowledge of therapeutic modalities and standardized functional outcome measures
• Ability to develop and implement measurable, timely goals
• Proficient documentation and electronic medical record use
• Familiarity with discharge planning and community resource coordination
• Strong patient and family education skills
