Roles & Responsibilities
- Provide comprehensive geriatric care across nursing facilities and clinic settings
- Lead and participate in the Advanced Care Coordination (Thrive) program to improve patient outcomes
- Teach, mentor, and supervise residents, medical students, and fellows in a supportive learning environment
- Collaborate with interdisciplinary teams to coordinate complex care plans and transitions
- Share call responsibilities with colleagues to ensure continuous patient coverage
- Contribute to clinical leadership and practice improvements that enhance patient-centered care
- Deliver high-quality documentation and participate in quality initiatives for geriatric populations
Qualifications
- Board Certified or Board Eligible in Geriatrics, Internal Medicine, or Family Medicine
- If Board Eligible, must become Board Certified within 2 years of becoming eligible
- Open to new graduates who have completed a Fellowship in Geriatrics
- Open to experienced candidates trained in Internal Medicine or Family Medicine
- Not accepting candidates with lapsed board certification who need to re-certify
- Not accepting visa candidates (no sponsorship) and not accepting locum tenens
- Willingness to be on-site and not seeking secondary employment (not open to commuters/secondary jobs)
Education
- Completion of a Geriatrics Fellowship preferred (new grads with fellowship encouraged)
- Training in Internal Medicine or Family Medicine (experienced candidates welcome)
- Board Certified or Board Eligible in Geriatrics/Internal Medicine/Family Medicine
Knowledge & Skills
- Expertise in geriatric patient care and management of complex, multi-morbid patients
- Experience providing nursing facility care and home-based care
- Advanced care coordination skills, including transition-of-care planning
- Teaching and precepting skills for residents, fellows, and students
- Strong team-based care and interdisciplinary collaboration abilities
- Clinical leadership and quality improvement experience
- Comfort with shared call models and collaborative coverage
