Roles & Responsibilities

• Provide comprehensive geriatric care across nursing facilities and clinic settings
• Lead and participate in the Advanced Care Coordination (Thrive) program to improve patient outcomes
• Teach, mentor, and supervise residents, medical students, and fellows in a supportive learning environment
• Collaborate with interdisciplinary teams to coordinate complex care plans and transitions
• Share call responsibilities with colleagues to ensure continuous patient coverage
• Contribute to clinical leadership and practice improvements that enhance patient-centered care
• Deliver high-quality documentation and participate in quality initiatives for geriatric populations

Qualifications

• Board Certified or Board Eligible in Geriatrics, Internal Medicine, or Family Medicine
• If Board Eligible, must become Board Certified within 2 years of becoming eligible
• Open to new graduates who have completed a Fellowship in Geriatrics
• Open to experienced candidates trained in Internal Medicine or Family Medicine
• Not accepting candidates with lapsed board certification who need to re-certify
• Not accepting visa candidates (no sponsorship) and not accepting locum tenens
• Willingness to be on-site and not seeking secondary employment (not open to commuters/secondary jobs)

Education

• Completion of a Geriatrics Fellowship preferred (new grads with fellowship encouraged)
• Training in Internal Medicine or Family Medicine (experienced candidates welcome)
• Board Certified or Board Eligible in Geriatrics/Internal Medicine/Family Medicine

Knowledge & Skills

• Expertise in geriatric patient care and management of complex, multi-morbid patients
• Experience providing nursing facility care and home-based care
• Advanced care coordination skills, including transition-of-care planning
• Teaching and precepting skills for residents, fellows, and students
• Strong team-based care and interdisciplinary collaboration abilities
• Clinical leadership and quality improvement experience
• Comfort with shared call models and collaborative coverage
