Roles & Responsibilities
- Conduct thorough speech-language evaluations to create clear, patient-centered plans
- Deliver daily treatments that improve communication, swallowing, and quality of life
- Coordinate and manage safe discharges, ensuring continuity of care
- Document clinical progress and maintain productivity expectations (90%) with accuracy
- Collaborate closely with the interdisciplinary team to deliver holistic patient care
- Represent the therapy team professionally (black scrubs dress code) and support facility operations
Qualifications
- Active New Mexico SLP license or eligibility to obtain state licensure
- 1-3 years of clinical experience; prior SNF experience strongly preferred
- Master's degree in Speech-Language Pathology (or equivalent clinical graduate degree)
- ASHA CCC-SLP preferred but not required
- Ability to follow Continuum Rehabilitation Group policies and productivity standards
- Willingness to be onsite to receive guaranteed hours and adhere to dress code
Education
- Master's degree in Speech-Language Pathology
Knowledge & Skills
- Skilled in evaluations, daily treatments, and discharge planning
- Experience with SNF workflows, documentation, and productivity tracking
- Strong clinical reasoning, time management, and patient-family communication
- Comfort working within an interdisciplinary therapy team
- Ability to adapt treatment plans for geriatric and medically-complex patients
