Roles & Responsibilities

• Assess and treat speech, language, cognitive-communication, and swallowing disorders for SNF residents
• Develop and implement individualized treatment plans that improve patient function and quality of life
• Document evaluations, treatment notes, and outcomes accurately and timely to meet facility standards
• Collaborate with the interdisciplinary team to coordinate care and support patient goals
• Maintain productivity expectations (90% productivity requirement) while delivering high-quality, patient-centered care
• Adhere to facility policies and Continuum Rehabilitation Group policies and procedures, including dress code and on-site requirements

Qualifications

• 1-3 years of SLP clinical experience (as noted in the posting)
• Active state SLP license or ability to obtain licensure promptly
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred
• Current CPR/basic life support certification as required by facility
• Strong documentation skills and the ability to meet productivity standards
• Reliable, professional demeanor with required on-site presence to receive guaranteed hours

Knowledge & Skills

• Skilled in evaluation and treatment of speech, language, cognition, and dysphagia
• Excellent documentation and clinical reasoning skills
• Ability to work effectively within a SNF therapy team
• Time management skills to balance caseload and productivity requirements
• Familiarity with facility policies and ability to follow CRG policies and procedures
• Professional communication skills with patients, families, and clinical staff
