Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to assigned residents, focusing on safety and dignity
• Administer medications and treatments accurately and document according to facility policy
• Monitor vital signs and clinical status, promptly reporting changes to the care team
• Collaborate with nurses, therapists, and providers to implement individualized care plans
• Perform wound care, ADL assistance, and supportive interventions to promote resident comfort
• Maintain clear, timely charting in the electronic medical record and communicate with families as appropriate
• Support a culture of infection control, patient safety, and continuous quality improvement

Qualifications

• Current LPN license in Rhode Island (or compact state eligibility as applicable)
• Graduate of an accredited LPN/Vocational Nursing program
• BLS (Basic Life Support) certification preferred or willing to obtain prior to start
• Comfortable administering medications, performing wound care, and documenting in EMR
• Ability to work evenings and every other weekend as required by the assignment
• Strong communication skills, teamwork mindset, and a patient-first attitude

Education

• Completion of an accredited LPN/LVN program (diploma or certificate)

Knowledge & Skills

• Medication administration and dosage calculation basics
• Skilled in taking and interpreting vital signs and observing clinical changes
• Familiarity with common long-term care procedures and documentation practices
• Good clinical judgement, time management, and prioritization under typical shift demands
• Effective interpersonal and communication skills with residents, families, and staff
• Infection control principles and safe patient handling techniques
