Roles & Responsibilities

• Provide compassionate, high-quality bedside care to residents and patients
• Administer medications and treatments accurately and on schedule
• Perform regular patient assessments and monitor vital signs, reporting changes promptly
• Document care thoroughly and maintain accurate clinical records
• Collaborate with RNs and the interdisciplinary care team to coordinate patient-centered plans
• Assist with activities of daily living and support patient comfort and dignity
• Follow infection control and safety protocols to protect patients and staff

Qualifications

• Active Licensed Practical Nurse (LPN) license (Pennsylvania) required
• Current Basic Life Support (BLS) certification required or obtained upon hire
• Previous LPN experience in a clinical or long-term care setting preferred
• Ability to work a rotating evening/night schedule and every other weekend
• Strong communication skills and a team-focused attitude

Education

• Graduate of an accredited LPN program
• High school diploma or GED plus completion of LPN training
• Active state LPN licensure

Knowledge & Skills

• Medication administration and safe documentation practices
• Patient assessment and vital sign monitoring
• Basic wound care and mobility assistance
• Familiarity with electronic medical records and charting
• Infection prevention, safety standards, and regulatory compliance
• Time management and prioritization in a fast-paced environment
• Teamwork and clear, compassionate communication with patients and families
