Roles & Responsibilities

• Provide compassionate, person-centered bedside care to residents and patients
• Administer medications and follow established care plans with accuracy and attention
• Monitor and document vital signs, clinical changes, and patient responses
• Perform nursing procedures and assist with activities of daily living as needed
• Communicate effectively with the interdisciplinary team and family members
• Maintain tidy, accurate electronic and paper documentation to support continuity of care
• Support a positive care environment by modeling professionalism and teamwork

Qualifications

• Active Licensed Practical Nurse (LPN) license — must be current and in good standing
• Current BLS (Basic Life Support) certification from a recognized provider
• Comfortable working 12+ hour day shifts (7:00 AM - 7:30 PM)
• Previous experience in long-term care, skilled nursing, or similar settings preferred
• Good communication skills, dependability, and a collaborative attitude

Education

• Completion of a state-approved Practical Nursing program and active LPN licensure

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic wound care, infection control, and ADL assistance
• Ability to accurately assess and report changes in patient condition
• Familiarity with electronic medical records and standard nursing documentation
• Strong teamwork, time management, and prioritization skills
