Roles & Responsibilities

• Provide compassionate, bedside nursing using LPN scope of practice to keep residents safe and comfortable
• Administer medications and treatments accurately and document them clearly
• Monitor patient status, report changes to the care team, and support care planning
• Collaborate with RNs and interdisciplinary staff to deliver consistent, high-quality care
• Support residents with activities of daily living while promoting dignity and independence
• Maintain accurate, timely charting and assist with shift handoffs to ensure continuity of care
• Adhere to facility policies and regulatory standards to protect patients and staff

Qualifications

• Current LPN license (active) — Pennsylvania licensure preferred
• Completion of an accredited practical nursing program
• Basic Life Support (BLS/CPR) certification preferred or willingness to obtain
• Clinical experience in long-term care, skilled nursing, or similar settings preferred
• Reliable, team-oriented, with strong communication and documentation habits

Education

• Graduate of an accredited LPN/LVN program or equivalent practical nursing diploma

Knowledge & Skills

• Solid fundamentals of practical nursing and medication administration
• Proficient in clinical documentation and electronic charting basics
• Good assessment skills and the ability to recognize changes in condition
• Strong interpersonal skills for working with patients, families and the care team
• Time management and prioritization for a busy night shift environment
