Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents, focusing on safety and dignity
• Administer medications and treatments accurately and on schedule
• Monitor, record, and report changes in patient condition to the care team
• Perform routine assessments, assist with ADLs, and promote patient mobility
• Collaborate with nurses, therapists, and physicians to deliver coordinated care
• Document clinical care thoroughly and complete clear handoffs at shift change
• Support resident and family education with empathy and clear communication

Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing (state of practice)
• Basic Life Support (BLS) certification (or willingness to obtain promptly)
• Comfortable working 12+ hour day shifts in a long-term care/skilled nursing environment
• Strong communication skills and the ability to work well on a multidisciplinary team
• Flexible, reliable, and committed to professional, patient-centered care

Education

• Completion of an accredited LPN/LVN program
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Skilled in vitals assessment, wound care basics, and ADL assistance
• Familiarity with electronic medical records and accurate charting
• Good clinical judgment, time management, and prioritization under busy conditions
• Ability to follow care plans and communicate changes effectively to the team
