Roles & Responsibilities

• Deliver compassionate bedside care and monitor patient condition during night shifts
• Administer medications and treatments accurately and document per policy
• Perform routine assessments and report significant changes to the care team
• Maintain thorough and timely charting in the electronic medical record
• Assist with wound care, ADLs and procedures as directed
• Collaborate with RNs, providers and interdisciplinary team members to support patient outcomes
• Provide clear patient and family education and emotional support

Qualifications

• Active LPN license in Pennsylvania (required)
• Current BLS/CPR certification (or ability to obtain prior to start)
• Comfortable working night shift hours and a rotating schedule
• Previous experience in long-term care, skilled nursing, or acute care preferred
• Strong clinical judgment, reliable documentation skills and a patient-first attitude

Education

• Graduated from an accredited Practical Nursing program
• Diploma or certificate qualifying for LPN licensure

Knowledge & Skills

• Medication administration and safe documentation practices
• Patient assessment and recognition of changes in condition
• Familiarity with electronic medical record systems and charting
• Infection control and standard precautions
• Effective communication and teamwork in a busy clinical setting
• Time management and the ability to prioritize during night shifts
