Roles & Responsibilities
- Provide compassionate, resident-centered nursing care to promote comfort and dignity
- Administer medications and treatments accurately and safely
- Perform routine assessments and document changes in condition promptly
- Collaborate with nurses, therapists, and interdisciplinary team members to implement care plans
- Educate residents and families about care, medications, and safety practices
- Assist with activities of daily living while encouraging resident independence
- Adhere to infection control, safety protocols, and facility policies to protect residents and staff
Qualifications
- Current Licensed Practical Nurse (LPN) credential in good standing
- Completion of an accredited LPN training program
- Previous experience in long-term care or rehab is a plus
- Strong interpersonal skills and the ability to work well on a multidisciplinary team
- Current BLS/CPR certification preferred or able to obtain
Education
- Graduate of an accredited LPN/Vocational Nursing program; high school diploma or GED required
Knowledge & Skills
- Medication administration and documentation best practices
- Basic patient assessment and charting skills
- Familiarity with long-term care/rehab workflows and resident care plans
- Comfortable using electronic health records and standard nursing documentation
- Good time management, prioritization, and communication skills
