Roles & Responsibilities

• Provide compassionate direct patient care, including medication administration and monitoring vital signs
• Perform skilled nursing tasks such as wound care, dressing changes, and catheter care with attention to safety
• Collaborate with the interdisciplinary team to implement individualized care plans and ensure continuity of care
• Document assessments, interventions, and patient responses accurately in the medical record
• Communicate clearly with patients and families, educating them on care plans and discharge instructions
• Respond promptly to changes in patient condition and escalate concerns to the appropriate provider

Qualifications

• Current Licensed Practical Nurse (LPN) license in the state of practice
• Basic Life Support (BLS) certification required; additional certifications encouraged
• Clinical experience in long-term care or skilled nursing preferred
• Current immunizations and ability to pass background and health screenings
• Comfort working 12+ hour day shifts and adapting to a fast-paced environment
• Strong communication skills and a patient-centered attitude

Education

• Graduate of an accredited Licensed Practical Nurse (LPN) program and current LPN license

Knowledge & Skills

• Medication administration and safe documentation practices
• Assessment and monitoring of patient condition, including reporting changes
• Wound care and basic procedural skills common in long-term care settings
• Familiarity with electronic health records and accurate charting
• Infection control principles and safe patient handling
• Time management, teamwork, and clear patient/family communication
