Roles & Responsibilities
- Provide skilled occupational therapy assistance in a skilled nursing facility to improve patient independence
- Support evaluations and implement individualized treatment plans under direction of an OTR
- Collaborate with interdisciplinary team members to coordinate care and promote positive outcomes
- Document patient progress clearly and timely in the medical record
- Educate residents and families on safe strategies, adaptive equipment, and carryover activities
- Adapt interventions for a variety of diagnoses typical to SNF populations and float between sites as needed
Qualifications
- Certified Occupational Therapy Assistant (COTA) credential required
- Eligible for or holding Ohio state licensure/certification as a COTA
- Open to all experience levels — new grads welcome (years experience: any)
- Comfortable floating to a sister location in Centerville when scheduled
- Strong communication skills and ability to work within an interdisciplinary team
- Reliable transportation for travel between Middletown and Centerville as required
Education
- Completion of an accredited Occupational Therapy Assistant (OTA) program
- Certified Occupational Therapy Assistant (COTA) credential
Knowledge & Skills
- Experience or familiarity with skilled nursing facility (SNF) practice setting
- Skilled in ADL/IADL training, adaptive equipment recommendations, and functional transfers
- Competent documentation and charting in facility medical records
- Ability to prioritize caseload and manage time efficiently
- Patient-centered approach with strong interpersonal and family-education skills
