Roles & Responsibilities
- Assess patients’ functional abilities and develop individualized occupational therapy plans
- Implement therapeutic interventions to improve independence with ADLs and daily routines
- Collaborate with an interdisciplinary team to coordinate patient-centered care
- Document evaluations, treatment plans and progress notes accurately and timely
- Educate patients and families on home programs, adaptive equipment and safety
- Monitor productivity targets and contribute to a high-performing therapy program
- Participate in team meetings and support facility initiatives to improve outcomes
Qualifications
- 1-3 years of occupational therapy experience preferred (SNF experience a plus)
- Occupational Therapy credential/qualification as required by state licensing (or eligible to obtain)
- Comfort following Continuum Rehabilitation Group (CRG) policies & procedures
- Ability to meet productivity expectations (90% productivity requirement noted)
- Professional presentation and adherence to facility dress code (black scrubs)
- Reliable presence on-site to receive guaranteed hours and support patient care
Knowledge & Skills
- Skilled Nursing Facility (SNF) clinical experience and geriatrics knowledge
- Strong patient assessment and ADL-focused treatment planning
- Proficient documentation skills and familiarity with productivity tracking
- Ability to work collaboratively with PT, SLP, nursing and facility staff
- Good communication skills with patients, families and the care team
- Comfortable with adaptive equipment recommendations and discharge planning
