Roles & Responsibilities

• Facilitate patient transfers across hospitals, coordinating acceptance with receiving physicians and specialists
• Perform case assessments after intake: collect vitals, clinical details, and provide concise patient reports
• Request and manage bed placement using TeleTracking and communicate bed needs with placement teams
• Document thoroughly in the EMR (Cerner or Epic) and maintain accurate, timely charts
• Coordinate multi-party communications using Tiger Connect, Google apps, and other messaging tools
• Manage multiple active cases simultaneously (often 10-15 at a time) while prioritizing safety and timeliness
• Refer out patients when appropriate (LVN-specific responsibility) and escalate as needed
• Follow clinical algorithms and apply strong clinical judgment to ensure appropriate patient routing

Qualifications

• Active RN or LVN license (eligible to submit license for assignment)
• BLS certification required
• Minimum of 2 years of ICU, ED, or transfer center experience required (posting lists 3 years of experience)
• Acute care and critical care assessment experience with sound clinical judgment
• Proven experience with Cerner or Epic charting systems
• Comfortable using multiple computer applications, strong multitasking and typing skills (typing test recommended)
• Excellent customer service and communication skills when interacting with clinicians, specialists, and bed placement teams
• Completion of the UNA Dys exam (or Relias Dys exam within last 12 months) is required prior to acceptance; candidates get two attempts and must provide proof of attempts
• Two supervisory references required at submittal (one within 12 months and one within 3 years)

Education

• Active RN or LVN license required
• BSN not required

Knowledge & Skills

• Proficient with Cerner and/or Epic EMR systems
• Experienced using TeleTracking for bed management and transfer coordination
• Familiar with Tiger Connect (secure clinician messaging) and Google apps for multi-chat communications
• Strong clinical assessment skills and critical care knowledge
• Excellent multitasking, computer proficiency, and rapid prioritization
• Skilled at clear, professional communication with physicians, specialists, and interfacility teams
• Customer-service oriented with the ability to remain calm during high-volume case management
