Roles & Responsibilities

• Conduct initial patient assessments and document findings clearly in Cerner
• Perform Utilization Review using Interqual criteria (training provided)
• Develop and execute discharge plans that safely coordinate care transitions
• Coordinate with multidisciplinary teams, hospitalists, RT and pharmacy to optimize patient care
• Advocate for patients with payers and connect uninsured/homeless patients to community resources
• Manage daily caseloads of approximately 15–20 consults, prioritizing care needs effectively
• Provide clear communication to families, care teams and external partners to ensure continuity

Qualifications

• Active Registered Nurse (RN) license — California required
• BSN required
• BLS certification required
• Minimum 2 years of clinical experience (per job posting)
• MUST have prior California Dignity experience — candidates without it will not be considered
• Two supervisory references required at submittal (one within 12 months and one within 3 years)
• Candidates may not have been directly employed by CommonSpirit/CHI/Dignity within the past year (PRN through outside firms acceptable)

Education

• BSN (Bachelor of Science in Nursing) — required
• Completed basic RN education and state licensure (California RN)

Knowledge & Skills

• Familiarity with case management and care coordination workflows
• Utilization Management and Interqual criteria (we will train in IQ as needed)
• Strong Cerner charting/documentation skills
• Experience dealing with payers and navigating care for uninsured or homeless populations
• Excellent collaboration, teamwork and communication skills
• Ability to reprioritize throughout the day and manage competing demands
