Roles & Responsibilities

• Provide direct patient care by assessing, diagnosing, and treating speech, language, and swallowing disorders in the home setting
• Develop and implement comprehensive, individualized plans of care in collaboration with patients, families, and the interdisciplinary team
• Document patient progress and outcomes clearly and timely to support continuity of care
• Use and troubleshoot rehabilitation equipment and communication aids to maximize patient independence
• Educate patients and caregivers on therapy goals, home programs, and strategies to promote carryover
• Work independently with minimal supervision while coordinating referrals and interdisciplinary communication
• Participate in orientation and required trainings to maintain high-quality, evidence-based practice

Qualifications

• Massachusetts Speech-Language Pathologist license (required)
• Certificate of Clinical Competence (CCC) required; Clinical Fellows accepted
• Master's degree or Doctorate in Speech-Language Pathology (required)
• Minimum 2 years SLP Home Care experience
• BLS (AHA) certification required
• Valid driver's license and own reliable vehicle (firm requirement)
• Ability to work independently, manage caseloads, and collaborate with interdisciplinary teams
• Acute care experience preferred

Education

• Master's degree or Doctorate in Speech-Language Pathology

Knowledge & Skills

• Strong clinical assessment and treatment planning skills for adult and geriatric populations
• Familiarity with rehabilitation equipment, language/communication aids, and wheelchairs
• Excellent documentation, time management, and patient education skills
• Comfortable navigating community settings and traveling between client homes
• Interdisciplinary collaboration and communication to support coordinated care
• Professionalism, clinical judgment, and patient-centered care approach
