Roles & Responsibilities
- Administer medications accurately and safely to residents during overnight hours
- Provide compassionate, person-centered resident care and support daily needs
- Monitor residents’ conditions and promptly report changes to the care team
- Maintain accurate documentation of medications, observations, and care provided
- Participate in Trinity specialty competency testing and any required trainings
- Support infection control practices and facility-required health screenings
Qualifications
- Active LPN license (license verification required)
- BLS certification (required)
- Experience with medication administration and resident care; overnight (non-sleeping) experience preferred
- Completion of Trinity specialty competency testing and required facility screenings
- Fit test and a clean Trinity 10-panel drug screen as part of onboarding
- Up-to-date immunizations or acceptable titers (COVID-19, MMR, Hepatitis B, Varicella, Influenza, Tdap) and physical/health statement
Education
- Licensed Practical Nurse (LPN) diploma or vocational nursing certificate
Knowledge & Skills
- Strong medication administration skills and attention to detail
- Knowledge of memory care best practices and dementia-sensitive approaches
- Excellent resident observation and assessment abilities
- Clear, compassionate communication with residents, families, and team members
- Familiarity with documentation standards and electronic or paper charting
- Ability to work independently during night shifts while collaborating with day teams
