Roles & Responsibilities
- Evaluate residents’ speech, language, cognitive-communication, and swallowing function with evidence-based assessments
- Develop and implement individualized treatment plans that improve communication and swallowing safety
- Document care thoroughly and timely in accordance with facility and CRG policies and productivity expectations
- Collaborate with interdisciplinary team members to coordinate care and support functional goals
- Monitor patient progress, adjust treatment plans as needed, and communicate outcomes to families and staff
- Maintain professional presence and dress code (black scrubs) while on duty
- Meet the therapist productivity requirement (90%) and uphold CRG policies and procedures
Qualifications
- Licensed Speech-Language Pathologist (state licensure required; New Mexico licensure required if practicing in NM)
- ASHA CCC-SLP preferred where applicable
- 1-3 years of clinical experience in a skilled nursing or long-term care setting preferred
- Comfortable working on-site and collaborating within a multidisciplinary therapy team
- Ability to meet productivity expectations and adhere to company policies
Knowledge & Skills
- Skilled in assessment and treatment of dysphagia and cognitive-communication disorders
- Familiarity with SNF/long-term care documentation and regulations
- Strong clinical reasoning and goal-driven treatment planning
- Good interpersonal and communication skills for patient, family, and team interactions
- Time management and organization to meet productivity targets and documentation timelines
