Roles & Responsibilities

• Evaluate residents’ speech, language, cognitive-communication, and swallowing function with evidence-based assessments
• Develop and implement individualized treatment plans that improve communication and swallowing safety
• Document care thoroughly and timely in accordance with facility and CRG policies and productivity expectations
• Collaborate with interdisciplinary team members to coordinate care and support functional goals
• Monitor patient progress, adjust treatment plans as needed, and communicate outcomes to families and staff
• Maintain professional presence and dress code (black scrubs) while on duty
• Meet the therapist productivity requirement (90%) and uphold CRG policies and procedures

Qualifications

• Licensed Speech-Language Pathologist (state licensure required; New Mexico licensure required if practicing in NM)
• ASHA CCC-SLP preferred where applicable
• 1-3 years of clinical experience in a skilled nursing or long-term care setting preferred
• Comfortable working on-site and collaborating within a multidisciplinary therapy team
• Ability to meet productivity expectations and adhere to company policies

Knowledge & Skills

• Skilled in assessment and treatment of dysphagia and cognitive-communication disorders
• Familiarity with SNF/long-term care documentation and regulations
• Strong clinical reasoning and goal-driven treatment planning
• Good interpersonal and communication skills for patient, family, and team interactions
• Time management and organization to meet productivity targets and documentation timelines
