Roles & Responsibilities
- Perform comprehensive speech and language evaluations for skilled nursing residents
- Provide daily individualized treatment to improve communication and swallowing function
- Plan and coordinate timely discharges and transitions of care
- Document clinical findings and progress thoroughly and accurately
- Collaborate with the interdisciplinary therapy team to maximize patient outcomes
- Adhere to Continuum Rehabilitation Group policies and facility procedures
- Maintain productivity standards and contribute to a positive therapy department culture
Qualifications
- SLP credential/qualification (Speech-Language Pathologist) appropriate for practice
- 1-3 years of clinical experience (job posting specifies 1-3 years)
- Prior SNF (skilled nursing facility) experience strongly preferred
- Ability to work onsite and follow facility dress code and policies
- Comfort with documentation, productivity expectations, and collaborative care
Knowledge & Skills
- Skilled in evaluations, daily treatment planning, and discharge processes
- Experience working in a skilled nursing facility environment
- Strong clinical documentation and time management skills
- Ability to collaborate with interdisciplinary teams and follow facility policies
- Familiarity with productivity targets and meeting performance expectations
