Roles & Responsibilities

• Perform comprehensive speech and language evaluations for skilled nursing residents
• Provide daily individualized treatment to improve communication and swallowing function
• Plan and coordinate timely discharges and transitions of care
• Document clinical findings and progress thoroughly and accurately
• Collaborate with the interdisciplinary therapy team to maximize patient outcomes
• Adhere to Continuum Rehabilitation Group policies and facility procedures
• Maintain productivity standards and contribute to a positive therapy department culture

Qualifications

• SLP credential/qualification (Speech-Language Pathologist) appropriate for practice
• 1-3 years of clinical experience (job posting specifies 1-3 years)
• Prior SNF (skilled nursing facility) experience strongly preferred
• Ability to work onsite and follow facility dress code and policies
• Comfort with documentation, productivity expectations, and collaborative care

Knowledge & Skills

• Skilled in evaluations, daily treatment planning, and discharge processes
• Experience working in a skilled nursing facility environment
• Strong clinical documentation and time management skills
• Ability to collaborate with interdisciplinary teams and follow facility policies
• Familiarity with productivity targets and meeting performance expectations
