Roles & Responsibilities
- Assess residents' speech, language, cognitive-communication, and swallowing function
- Develop and implement individualized therapy plans that help residents regain independence
- Provide skilled therapy interventions and progress residents toward measurable goals
- Document assessments, plans, and progress clearly and timely according to facility policy
- Collaborate with nursing, rehab team members, and families to coordinate care
- Maintain productivity expectations (90% productivity requirement noted) while preserving quality
- Follow Continuum Rehabilitation Group policies and procedures and represent the facility professionally
- Be present on-site as required to receive guaranteed hours and ensure smooth patient care
Qualifications
- Primary SLP credential/role as listed (Speech-Language Pathologist)
- 1-3 years of clinical experience in therapy settings (per job posting: 1-3 years)
- Experience working in a Skilled Nursing Facility (SNF) or similar long-term care setting preferred
- Active state licensure or ability to obtain licensure for the state of New Mexico (verify facility requirements)
- Strong documentation skills and ability to meet productivity expectations
- Professionalism: able to follow facility dress code (black scrubs) and organizational policies
- Good interpersonal and communication skills for teamwork and family education

- Skilled nursing facility (SNF) therapy practices and workflow
- Assessment and treatment of dysphagia and cognitive-communication disorders
- Care plan development and measurable goal writing
- Accurate clinical documentation and productivity tracking
- Interdisciplinary collaboration with nursing and rehab team members
- Time management and ability to prioritize caseloads
