Roles & Responsibilities

• Assess residents' speech, language, cognitive-communication, and swallowing function
• Develop and implement individualized therapy plans that help residents regain independence
• Provide skilled therapy interventions and progress residents toward measurable goals
• Document assessments, plans, and progress clearly and timely according to facility policy
• Collaborate with nursing, rehab team members, and families to coordinate care
• Maintain productivity expectations (90% productivity requirement noted) while preserving quality
• Follow Continuum Rehabilitation Group policies and procedures and represent the facility professionally
• Be present on-site as required to receive guaranteed hours and ensure smooth patient care

Qualifications

• Primary SLP credential/role as listed (Speech-Language Pathologist)
• 1-3 years of clinical experience in therapy settings (per job posting: 1-3 years)
• Experience working in a Skilled Nursing Facility (SNF) or similar long-term care setting preferred
• Active state licensure or ability to obtain licensure for the state of New Mexico (verify facility requirements)
• Strong documentation skills and ability to meet productivity expectations
• Professionalism: able to follow facility dress code (black scrubs) and organizational policies
• Good interpersonal and communication skills for teamwork and family education

Knowledge & Skills

• Skilled nursing facility (SNF) therapy practices and workflow
• Assessment and treatment of dysphagia and cognitive-communication disorders
• Care plan development and measurable goal writing
• Accurate clinical documentation and productivity tracking
• Interdisciplinary collaboration with nursing and rehab team members
• Time management and ability to prioritize caseloads
