Roles & Responsibilities

• Evaluate residents and develop individualized treatment plans that improve mobility and independence
• Deliver skilled therapeutic interventions and progress patients toward measurable goals
• Document assessments, plans, and progress notes accurately and timely
• Collaborate with nursing and interdisciplinary team members to coordinate care
• Meet facility productivity expectations and contribute to a high-performing therapy program
• Follow Continuum Rehabilitation Group policies and facility protocols, including dress code and on-site presence
• Participate in team meetings and communicate effectively with families and caregivers

Qualifications

• Doctor of Physical Therapy (DPT) or appropriate Physical Therapist degree
• 1-3 years of relevant experience (SNF experience a plus)
• Valid Physical Therapist license (or ability to obtain licensure) for New Mexico
• Strong clinical reasoning, assessment, and documentation skills
• Comfort working within SNF practice setting and meeting productivity expectations

Education

• DPT or equivalent Physical Therapy degree

Knowledge & Skills

• Skilled Nursing Facility (SNF) therapy workflows and documentation practices
• Gait training, transfers, balance and fall prevention strategies
• Therapeutic exercise prescription and adaptive equipment recommendations
• Clear patient and family communication with a team-centered approach
• Time management and productivity in an outpatient/inpatient rehab environment
