Roles & Responsibilities

• Provide compassionate, high-quality bedside care to residents, ensuring safety and comfort
• Administer medications and document accurately in the patient record
• Perform routine assessments and promptly report changes in condition
• Collaborate with RNs, physicians, and interdisciplinary team members for coordinated care
• Maintain clear, timely charting and handoffs to support continuity of care
• Support admission, transfer, and discharge processes with attention to detail
• Participate in unit-based initiatives to improve patient outcomes and experience

Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing
• Current Basic Life Support (BLS/CPR) certification preferred or must obtain upon hire
• Comfortable working night shifts and every-other-weekend schedule
• Strong communication skills and a team-focused attitude
• Clinical experience in long-term care, skilled nursing, or similar settings is a plus

Education

• Completion of an accredited LPN/Vocational Nursing program

Knowledge & Skills

• Solid skills in medication administration and basic clinical assessments
• Familiarity with charting and electronic medical records
• Infection control and wound care fundamentals
• Ability to prioritize tasks and respond to changes in patient condition
• Compassionate patient interaction and family communication
