Roles & Responsibilities
- Provide compassionate direct patient care and assist with activities of daily living (ADLs)
- Monitor and document vital signs, reporting changes promptly to nursing staff
- Assist with safe transfers, mobility and use of assistive devices to support patient safety
- Support rehabilitation goals in collaboration with nurses and therapy teams at Canyon Trans Rhb Ctr
- Maintain infection control standards and keep patient areas clean and safe
- Accurately document care in patient records and communicate clearly with the care team
- Build trusting relationships with patients and families, preserving dignity and respect
Qualifications
- Active Certified Nursing Assistant (CNA) certification in good standing
- CPR/BLS certification preferred or willingness to obtain
- Experience in rehab or long-term care is a plus; new grads encouraged to apply
- Able to work scheduled 12-hour day shifts (Friday, Saturday, Sunday)
- Must meet facility-required background and immunization policies
Education
- High school diploma or equivalent and completion of an accredited CNA program
Knowledge & Skills
- Strong patient-centered care skills and assistance with ADLs
- Accurate vital signs measurement and clinical documentation
- Safe transfer and mobility techniques
- Infection prevention and proper PPE use
- Clear communication, teamwork and the ability to follow care plans
