Roles & Responsibilities

• Provide compassionate, hands-on personal care and assistance with activities of daily living
• Safely assist with mobility, transfers, and positioning to promote patient comfort and safety
• Accurately document care, observations, and changes in patient condition in the chart
• Communicate clearly and respectfully with patients, families, and interdisciplinary team members
• Follow individualized care plans and facility policies to ensure high-quality care
• Maintain infection control, safety standards, and a clean, welcoming environment for residents

Qualifications

• Current LNA certification (Class T-LNA) — required
• Comfortable working night shifts and providing weekend coverage as scheduled
• Previous direct care experience in long-term care, assisted living, or similar settings preferred
• Strong communication skills and ability to work collaboratively with a care team
• Valid identification and ability to complete facility onboarding and background checks

Education

• High school diploma or GED preferred; completion of an approved LNA training program required

Knowledge & Skills

• Skilled in ADLs, toileting, bathing, grooming, and feeding assistance
• Knowledge of basic infection control practices and patient safety protocols
• Basic charting and documentation skills with attention to detail
• Ability to recognize and report changes in patient condition promptly
• Time management and prioritization in a fast-paced care environment
• Compassionate bedside manner and strong interpersonal skills
