Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in a rehabilitation setting
• Administer medications and treatments safely and accurately, following facility policies
• Perform routine assessments and monitor changes in condition, escalating concerns promptly
• Document care thoroughly in the electronic medical record to ensure continuity and quality
• Collaborate with nurses, therapists, and the interdisciplinary team to support care plans
• Assist residents with activities of daily living and promote functional independence
• Adhere to infection control standards and safety protocols to protect patients and staff
• Support discharge planning and communicate with families to ensure smooth transitions

Qualifications

• Current Licensed Practical Nurse (LPN) license valid in New Mexico
• Current BLS (Basic Life Support) certification or willingness to obtain prior to start
• Previous experience in long-term care, rehabilitation, or skilled nursing preferred
• Strong clinical judgment, dependable attendance, and ability to work night shifts
• Clear communication skills and a collaborative attitude to work effectively with the care team

Education

• Completion of an accredited LPN/LVNs program and active LPN license

Knowledge & Skills

• Skilled in medication administration and routine nursing procedures
• Comfortable with resident assessment, reporting, and EMR documentation
• Knowledge of infection control practices and patient safety standards
• Ability to prioritize tasks, manage time during busy shifts, and respond calmly in emergencies
• Good interpersonal skills for building rapport with residents and families
