Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents in a rehabilitation setting
- Administer medications and treatments safely and accurately, following facility policies
- Perform routine assessments and monitor changes in condition, escalating concerns promptly
- Document care thoroughly in the electronic medical record to ensure continuity and quality
- Collaborate with nurses, therapists, and the interdisciplinary team to support care plans
- Assist residents with activities of daily living and promote functional independence
- Adhere to infection control standards and safety protocols to protect patients and staff
- Support discharge planning and communicate with families to ensure smooth transitions
Qualifications
- Current Licensed Practical Nurse (LPN) license valid in New Mexico
- Current BLS (Basic Life Support) certification or willingness to obtain prior to start
- Previous experience in long-term care, rehabilitation, or skilled nursing preferred
- Strong clinical judgment, dependable attendance, and ability to work night shifts
- Clear communication skills and a collaborative attitude to work effectively with the care team
Education
- Completion of an accredited LPN/LVNs program and active LPN license
Knowledge & Skills
- Skilled in medication administration and routine nursing procedures
- Comfortable with resident assessment, reporting, and EMR documentation
- Knowledge of infection control practices and patient safety standards
- Ability to prioritize tasks, manage time during busy shifts, and respond calmly in emergencies
- Good interpersonal skills for building rapport with residents and families
