Physical Therapist Assistant
Location: Chagrin Falls, OH
Contract Type: Per-Diem
Candidate Pay Rate: $27 - $29 per hour

Roles & Responsibilities
- Provide physical therapy treatments under the supervision of a licensed Physical Therapist to help residents regain strength and mobility
- Assist residents with therapeutic exercises and mobility training, promoting independence and safety
- Monitor patient progress and report meaningful updates to the supervising PT to inform care plans
- Maintain accurate, timely, and professional documentation to support quality care and compliance
- Collaborate with the interdisciplinary healthcare team to coordinate patient-centered treatment goals
- Communicate clearly and compassionately with residents and families to encourage participation and progress
Qualifications
- Active Ohio PTA License (required)
- Current BLS (Basic Life Support) certification
- SNF experience preferred — experience in skilled nursing is a plus
- Strong clinical judgment and hands-on patient care skills
- Excellent communication and teamwork abilities
- Willingness to work PRN/per diem shifts and adapt to facility scheduling needs
Knowledge & Skills
- Skilled in therapeutic exercise prescription and mobility training
- Proficient with patient assessment and progress monitoring
- Strong documentation practices and attention to detail
- Ability to collaborate effectively with nurses, therapists, and care staff
- Knowledge of safety and fall-prevention strategies in elder care
- Compassionate patient-centered care and strong interpersonal skills
