Location: Chagrin Falls, OH

Contract Type: Per-Diem

Candidate Pay Rate: $27 - $29 per hour

Roles & Responsibilities

Provide physical therapy treatments under the supervision of a licensed Physical Therapist to help residents regain strength and mobility
Assist residents with therapeutic exercises and mobility training, promoting independence and safety
Monitor patient progress and report meaningful updates to the supervising PT to inform care plans
Maintain accurate, timely, and professional documentation to support quality care and compliance
Collaborate with the interdisciplinary healthcare team to coordinate patient-centered treatment goals
Communicate clearly and compassionately with residents and families to encourage participation and progress

Qualifications

Active Ohio PTA License (required)
Current BLS (Basic Life Support) certification
SNF experience preferred — experience in skilled nursing is a plus
Strong clinical judgment and hands-on patient care skills
Excellent communication and teamwork abilities
Willingness to work PRN/per diem shifts and adapt to facility scheduling needs

Knowledge & Skills

Skilled in therapeutic exercise prescription and mobility training
Proficient with patient assessment and progress monitoring
Strong documentation practices and attention to detail
Ability to collaborate effectively with nurses, therapists, and care staff
Knowledge of safety and fall-prevention strategies in elder care
Compassionate patient-centered care and strong interpersonal skills
