Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents, ensuring safety and dignity
• Administer medications and treatments accurately and document thoroughly
• Monitor patient status, report changes to the interdisciplinary team, and escalate concerns
• Collaborate with RNs and other staff to implement care plans that improve outcomes
• Perform timely and accurate charting in the facility’s documentation system
• Support patient education and family communication with clarity and empathy
• Participate in shift handoffs and ensure continuity of care across the rotation

Qualifications

• Valid LPN license for Pennsylvania (required) — bring that clinical expertise
• Current BLS (Basic Life Support) / CPR certification (required or to be obtained promptly)
• Prior long-term care or skilled nursing experience preferred
• Comfortable working evening/swing shifts and providing weekend coverage
• Strong communication, teamwork, and time-management skills

Education

• LPN diploma or equivalent vocational/technical nursing education

Knowledge & Skills

• Medication administration and basic nursing procedures
• Resident assessment, vital signs, and documentation best practices
• Familiarity with electronic medical/health record systems
• Infection control, wound care basics, and safe patient handling
• Ability to triage and escalate clinical concerns appropriately
