Roles & Responsibilities

• Administer medications and treatments safely and accurately according to facility policy
• Monitor and record patient vital signs and report changes to the care team
• Provide direct bedside care and assist with activities of daily living with compassion
• Document patient status, interventions and outcomes thoroughly and timely
• Collaborate with RNs, physicians and interdisciplinary staff to deliver coordinated care
• Support patient and family education, answering questions and offering clear guidance
• Promote a safe, clean environment and follow infection control protocols

Qualifications

• Current LPN license (practical nurse) in Pennsylvania or eligible to obtain one
• Basic Life Support (BLS/CPR) certification required or ability to obtain prior to start
• Ability to pass background check and drug screening as required by the facility
• Comfortable working in a fast-paced clinical environment and collaborating with a team
• Strong communication skills and patient-centered mindset

Education

• Graduate of an accredited Practical Nursing / LPN program
• High school diploma or GED

Knowledge & Skills

• Proficient in medication administration and basic nursing procedures
• Skilled in taking and interpreting vital signs and patient assessments
• Clear and accurate clinical documentation
• Infection prevention and safety practices
• Teamwork, time management and the ability to prioritize patient needs
• Compassionate patient interaction and family communication
