Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse
• Administer medications and treatments accurately and safely
• Monitor resident condition and report changes to the interdisciplinary team
• Document assessments, interventions and response to treatment in the medical record
• Support patient and family education with patience and clear communication
• Collaborate with nurses, therapists and support staff to deliver person-centered care
• Promote a safe, clean environment and adhere to infection prevention protocols

Qualifications

• Current Licensed Practical Nurse (LPN) credential — you’re a pro with a license
• Active LPN license in Vermont or ability to obtain state licensure prior to assignment
• Basic Life Support (BLS/CPR) certification (current)
• Prior long-term care or skilled nursing experience preferred
• Strong communication skills and a team-first attitude

Education

• Graduate of an accredited LPN/Licensed Practical Nurse program

Knowledge & Skills

• Medication administration and documentation best practices
• Resident assessment and monitoring of vitals
• Wound care fundamentals and basic dressing changes
• Familiarity with electronic medical records and charting
• Time management and prioritization on 12+ hour shifts
• Ability to work collaboratively within an interdisciplinary care team
