Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse
- Administer medications and treatments accurately and safely
- Monitor resident condition and report changes to the interdisciplinary team
- Document assessments, interventions and response to treatment in the medical record
- Support patient and family education with patience and clear communication
- Collaborate with nurses, therapists and support staff to deliver person-centered care
- Promote a safe, clean environment and adhere to infection prevention protocols
Qualifications
- Current Licensed Practical Nurse (LPN) credential — you’re a pro with a license
- Active LPN license in Vermont or ability to obtain state licensure prior to assignment
- Basic Life Support (BLS/CPR) certification (current)
- Prior long-term care or skilled nursing experience preferred
- Strong communication skills and a team-first attitude
Education
- Graduate of an accredited LPN/Licensed Practical Nurse program
Knowledge & Skills
- Medication administration and documentation best practices
- Resident assessment and monitoring of vitals
- Wound care fundamentals and basic dressing changes
- Familiarity with electronic medical records and charting
- Time management and prioritization on 12+ hour shifts
- Ability to work collaboratively within an interdisciplinary care team
