Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse
• Administer medications and document treatments accurately and timely
• Monitor resident status and communicate changes to the care team for optimal outcomes
• Support daily living activities, hygiene, and comfort measures with dignity and respect
• Collaborate with RN leadership and interdisciplinary team to implement care plans
• Maintain clear, professional documentation and follow facility policies and procedures
• Assist with admissions, discharges, and transfers as needed to ensure smooth transitions

Qualifications

• Current Licensed Practical Nurse (LPN) license; Pennsylvania license strongly preferred
• Ability to work five 8-hour shifts per week, including every other weekend, no exceptions
• Minimum competence in medication administration and basic nursing assessments
• Strong communication skills and a team-oriented mindset
• Reliable, professional, and able to adapt to facility protocols and schedules

Education

• Completion of an accredited Practical Nursing program (LPN diploma or equivalent)

Knowledge & Skills

• Skilled in basic nursing care, vital signs, and wound observation
• Comfortable with medication administration and accurate charting
• Able to recognize and report changes in patient condition promptly
• Good time management and prioritization on a busy unit
• Approachable bedside manner and strong patient advocacy
