Roles & Responsibilities
- Provide compassionate, patient-centered nursing care in a long-term/rehab setting
- Administer medications and follow physician orders accurately and safely
- Monitor and document vital signs, patient status and changes in condition
- Perform wound care, assist with activities of daily living and support mobility goals
- Communicate clearly with patients, families and the interdisciplinary care team
- Maintain accurate electronic medical records and complete timely documentation
- Participate in care planning and support discharge or rehab goals
- Promote a safe environment through infection control and facility protocols
Qualifications
- Current Licensed Practical Nurse (LPN) license (New Hampshire or eligible for NH licensing)
- BLS/CPR certification (current)
- Experience or interest in long-term care, rehabilitation, or skilled nursing preferred
- Up-to-date immunizations and ability to meet facility health requirements
- Strong communication, teamwork and patient advocacy skills
Education
- Graduate of an accredited practical nursing program or equivalent
Knowledge & Skills
- Medication administration and basic nursing procedures
- Patient assessment and monitoring
- Wound care basics and skin integrity awareness
- Electronic Medical Records (EMR) documentation
- Infection control practices and safety protocols
- Time management and prioritization in a busy care setting
- Compassionate patient and family education
