Roles & Responsibilities

• Provide compassionate, hands-on care to residents including assistance with activities of daily living
• Monitor and record vital signs and changes in condition accurately and promptly
• Support safe patient transfers and mobility with attention to dignity and comfort
• Communicate clearly with nurses, families, and the care team to coordinate excellent care
• Maintain clean, safe environments and follow infection control protocols
• Document care activities thoroughly and follow care plans and facility policies

Qualifications

• Current Licensed Nursing Assistant (LNA) certification for Vermont (VT) and/or New Hampshire (NH) as applicable
• Valid CPR certification or willingness to obtain if required by the facility
• Reliable transportation for local commuting to the facility
• Ability to work night shifts and every other weekend as required
• Strong communication skills and a compassionate, patient-centered approach

Knowledge & Skills

• Skilled in assisting with ADLs: bathing, dressing, feeding, toileting, and mobility
• Able to take and document vital signs and observe changes in condition
• Knowledge of basic infection control and safety practices
• Comfortable with routine documentation and electronic or paper charting
• Good interpersonal skills for teamwork and family communication
